Service Description
Our Enhanced Care Plan is a weekly wellness visit and is best for clients recovering from surgery, managing complex health needs, or needing close monitoring. During this visit, your Concierge RN will assess your overall health, review symptoms, check vital signs, evaluate medications, and provide recommendations to support your safety and wellbeing. Benefits of this service include: continuous oversight with rapid intervention, weekly vitals/wound checks/medication review, accountability and personalized health support, dramatically reduced unnecessary hospital visits, and helps build a strong proactive care relationship. Weekly RN support gives clients the fastest response to health changes and the highest level of prevention and safety.
